Botox for men—often called Brotox—uses the same botulinum toxin treatment used to soften dynamic facial lines. What changes is not the product because the patient is male, but the way the individual face may need to be assessed and treated.
Muscle activity, brow position, forehead anatomy, facial proportions and the amount of natural movement a patient wants to preserve can all influence treatment planning.

Quick answer: Brotox is not a different form of Botox. Good treatment is about understanding the individual male face—what is creating the concern, what should be softened and what should be preserved.

What Is Brotox?
“Brotox” is an informal term for Botox treatment in men.
BOTOX® Cosmetic contains onabotulinumtoxinA, which temporarily reduces activity in selected muscles responsible for dynamic expression lines. Common upper-face concerns include:
- vertical frown lines between the eyebrows;
- horizontal forehead lines; and
- crow’s feet around the outer eyes.
The objective should not automatically be to eliminate every visible line.
For many men, a more appropriate goal is to soften an expression that appears unintentionally tense, tired or stern while retaining recognisable facial movement and individual character.
A completely motionless forehead is therefore not necessarily a better result than one in which distracting lines have softened but appropriate expression remains.
Is Botox Different for Men and Women?
The medication does not work through a different mechanism because someone is male.
Treatment planning, however, may differ.
Research examining neuromodulator treatment in men describes population-level differences in facial anatomy, muscle mass and contraction patterns that can influence assessment, placement and dose.
These are tendencies—not rules.
Two men of the same age can have very different facial anatomy and muscle activity, just as two women can. Gender should therefore be regarded as one consideration within a much broader facial assessment rather than as a dosing formula.
The face in front of the doctor matters more than a generic idea of what “men’s Botox” should look like.
What Changes When Botox Is Planned for a Male Face?
Several considerations become particularly important.
Muscle Activity and Contraction Pattern
Facial lines are created by muscles that do not behave identically in every person.
One patient may have strong glabellar contraction producing deep vertical frown lines. Another may recruit the forehead heavily to maintain brow position. A third may show significant movement with relatively little visible creasing at rest.
Treatment therefore begins by looking at the face at rest and during expression.
Muscle strength and movement pattern are more clinically useful than simply assuming that a man requires a particular dose.
Brow Position and Forehead Anatomy
The forehead and eyebrows function together.
The frontalis muscle raises the brows as well as creating horizontal forehead lines. Treatment planning therefore needs to consider where the brows naturally sit and how much the patient relies on forehead activity to maintain their position.
The objective is not to manufacture a stereotypically “male” eyebrow.
It is to preserve an individual’s natural brow character while addressing the lines or expression that concern him.
Facial Proportions and Individual Character
Aesthetic treatment should consider the face as a whole.
A prominent brow, angular lower face, wider jaw or strong facial structure may be an important part of an individual’s appearance. Changing muscle activity in one region without considering the rest of the face can affect more than the wrinkle itself.
Successful male facial aesthetics is therefore partly about deciding what should not be changed.
How Much Movement Should Remain?
Not every patient wants the same endpoint.
Some prefer greater softening of their expression lines. Others want a very subtle change and are comfortable retaining visible forehead movement.
That conversation should happen before treatment.
The better question is not simply:
“How smooth can we make the forehead?”
It is:
“How much change will address the concern while still feeling natural to this face?”

Why Do Men Choose Botox?
Not every man seeking Botox wants to “look younger.”
Common consultation concerns may be expressed more simply:
“I look more tired than I feel.”
“People think I am angry when I am concentrating.”
“The lines between my brows have become much stronger.”
“I want to look refreshed without looking obviously treated.”
Botox should not be presented as necessary for professional success, social confidence or staying relevant.
It is an elective medical aesthetic treatment that can be considered when a patient wants to address an appropriate movement-related facial concern and clinical assessment suggests that botulinum toxin is a suitable way to do so.

Common Botox Treatment Areas for Men
Frown Lines Between the Brows
Vertical lines between the eyebrows are produced primarily by repeated activity of the glabellar muscle complex.
In some men, strong contraction can create a persistent stern or tense appearance.
Botulinum toxin can reduce selected muscle activity and soften these lines, while treatment planning should consider how the glabellar muscles interact with the surrounding forehead and brow.
Forehead Lines
Horizontal forehead lines form through repeated frontalis activity.
Because the frontalis also elevates the eyebrows, treating forehead lines requires more than simply targeting every visible crease.
How much the patient relies on this muscle to maintain brow position should form part of the assessment.
Crow’s Feet
Crow’s feet are the expression lines radiating from the outer eye area and are produced largely through orbicularis oculi activity.
Treatment can soften movement-related lines while aiming to preserve a natural smile and eye expression.
When Botox May Not Be the Right Treatment
A useful Botox consultation should establish not only where Botox can help, but also when another approach may be more appropriate.
Botox primarily addresses concerns caused by muscle activity.
It may therefore be less suitable as a standalone answer when the main concern is:
- deep static creasing that remains prominent even when the muscle is fully at rest;
- under-eye hollowing or facial volume loss;
- pigmentation or pigment-driven dark circles;
- significant skin laxity or tissue descent;
- skin texture or skin-quality change rather than muscle movement; or
- jaw width caused primarily by bone structure or fat, rather than enlarged masseter muscle.
Botulinum toxin can sometimes improve the dynamic component of an established line without completely erasing the skin crease that has developed over time.
Similarly, it cannot replace lost facial volume or function as a general skin-tightening treatment.
The first clinical decision is therefore not how much Botox to use. It is whether Botox is the right treatment for the concern at all.
Why Male Botox Is About Brow Position—Not Just Forehead Lines
One of the most important aspects of upper-face treatment is that forehead lines cannot be considered independently of the eyebrows.
The frontalis muscle that creates horizontal forehead lines is also the primary elevator of the brows.
If treatment concentrates only on smoothing the forehead without assessing brow position and muscle recruitment, the resulting balance may not suit the individual face.
Published research on male neuromodulator treatment specifically highlights the importance of frontalis assessment, dose and placement in avoiding unwanted changes such as brow heaviness or ptosis.
This does not mean that every man requires the same brow shape.
It means that natural brow position and the interaction between forehead and brow muscles should be understood before treatment.

Will Botox Make a Man Look Frozen or Feminine?
Botox itself does not make a face look feminine.
However, inappropriate treatment can alter facial character.
Excessive smoothing, unsuitable changes in brow shape or poor balance between neighbouring muscle groups may create an outcome that does not match the patient’s anatomy or treatment goals.
A better objective is:
Soften what is distracting while preserving what defines the face.
For some male patients, that means retaining visible forehead movement. For others, greater line reduction may be appropriate.
Neither approach should be imposed simply because the patient is male.
And no responsible treatment should promise to be completely “undetectable.”
A natural-looking result is better understood as one that remains balanced, proportionate and appropriate to the individual face.
Do Men Need More Botox Units?
Not automatically.
Male-specific research does suggest that stronger or larger muscles in some men can influence dosing requirements. A 2026 systematic review and meta-analysis of aesthetic treatments in men also found evidence that higher botulinum toxin doses improved outcomes in the male populations analysed.
That does not mean every man needs a higher dose.
Appropriate dosing should consider:
- the muscle being treated;
- muscle strength and contraction pattern;
- facial anatomy;
- brow position;
- previous treatment response, where relevant; and
- how much movement the patient wishes to preserve.
There should therefore be no universal “male dose” and no automatic percentage increase simply because the patient is a man.

Does Botox Cost More for Men?
There should not be a separate “male tariff” for BOTOX®.
The final treatment cost depends on factors such as the areas being treated, individual muscle activity, anatomy and the amount of product required.

A man with stronger facial muscles may require a different treatment plan from another patient—but this should emerge from clinical assessment rather than from gender-based pricing.

Masseter Botox in Men: Is a Slimmer Jaw Always Better?
No—and this is an important distinction.
The masseter is a powerful chewing muscle at the angle of the jaw. When substantial masseter hypertrophy contributes to lower-face width, botulinum toxin can reduce muscle activity and may gradually reduce muscular bulk.
That can create a narrower-looking lower face.
But a narrower jaw is not automatically a better aesthetic result for every man.
A broader or more angular lower face may be an important part of the individual’s proportions and facial character.
Jaw width may also arise from bone structure, fat distribution or other anatomical factors rather than the masseter muscle itself.
Before considering treatment, the more useful questions are:
Is the width actually muscle-driven?
and:
Would reducing that muscle improve the overall facial balance?

What Happens During a Male Botox Consultation?
A good consultation involves considerably more than selecting which wrinkles to inject.
Assessment at Rest
The doctor evaluates facial proportions, symmetry, brow position, forehead anatomy, established lines and the relationship between the eyebrows and upper eyelids where relevant.
Dynamic Muscle Assessment
You may be asked to frown, raise your eyebrows, smile or make other facial expressions.
This allows muscle strength and movement patterns to be observed rather than inferred.
Discussion of the Desired Endpoint
Questions may include:
- Which facial feature actually bothers you?
- Would you prefer maximum softening or greater retained movement?
- Do you feel you look tired, tense or stern?
- Which aspects of your appearance do you specifically not want changed?
- Have you had botulinum toxin previously, and if so, how did you respond?
Individual Treatment Planning
Only after this assessment should the appropriate treatment areas and dosing strategy be decided.
This is what separates individual facial treatment from a pre-designed “Brotox package.”
What Should a Man Expect on Treatment Day?
Once a treatment plan has been agreed, the selected areas are identified and small amounts of botulinum toxin are injected using a fine needle.
The injection component is generally brief.
Temporary redness, small injection marks, local tenderness or bruising can occur, but extensive recovery is not usually expected after an uncomplicated cosmetic treatment.
Your doctor should explain the aftercare appropriate to the areas treated and your individual circumstances.
Rather than following generic internet rules after an injectable procedure, follow the specific instructions provided by your treating doctor.
When Do Botox Results Appear and How Long Do They Last in Men?
Botox does not produce its full effect immediately.
For BOTOX® Cosmetic treatment of glabellar lines, the official BOTOX® Cosmetic prescribing information states that the injected muscles typically begin to show chemical denervation one to two days after injection, with the effect increasing during the first week.
The same prescribing information describes an approximate 3–4 month duration of effect for glabellar lines.
This should not be interpreted as an exact timeline for every patient or every treatment area.
Duration can vary according to:
- treatment area;
- muscle activity;
- individual response;
- dose and treatment plan; and
- previous treatment history.
There is no good reason to assume that every man will have exactly the same duration—or that treatment should automatically be repeated according to a fixed calendar.
Repeat treatment is better considered after return of muscle activity and clinical reassessment.
Is Botox Safe for Men?
The fundamental safety considerations for botulinum toxin treatment are not determined by gender.
BOTOX® Cosmetic is a prescription medicine and should be used after appropriate medical assessment.
Temporary injection-site effects such as redness, tenderness or bruising can occur. Depending on the area treated, unwanted effects including asymmetry, brow heaviness or eyelid/brow ptosis are also possible.
The official prescribing information lists contraindications including hypersensitivity to botulinum toxin preparations or formulation components and infection at the proposed injection site. It also contains warnings and precautions relating to spread of toxin effect and patients with certain pre-existing medical conditions.
Patients should tell their treating doctor about relevant medical conditions, medicines and previous botulinum toxin treatment before proceeding.
Unexpected difficulty swallowing, speaking or breathing or widespread muscle weakness after treatment requires urgent medical attention.
Discussing these risks does not mean that complications are expected.
It means that an elective injectable treatment should still be approached as a medical procedure.

The popularity of the word “Brotox” can make male Botox sound like a standardised product.
It should not be.
A result depends not only on the toxin used, but on understanding facial anatomy, muscle function, placement, dose and how changing one muscle group may influence another.

At What Age Should Men Start Botox?
There is no fixed age at which a man “should” start Botox.
Suitability depends on the concern being treated, facial movement, the development of dynamic lines, anatomy, treatment objectives and clinical assessment.
Age alone is not an indication for treatment.
Is There Downtime After Botox for Men?
Extensive downtime is not normally expected after an uncomplicated cosmetic treatment, although temporary redness, injection marks, tenderness or bruising can occur.
Follow the individual aftercare instructions given by your treating doctor.
Do Men Need More Botox Units?
Some men have stronger or larger facial muscles and may require a different dose, but there is no universal male dose.
Treatment should be based on the muscle being treated, its strength, anatomy and the intended amount of movement reduction.
Will Botox Make a Man’s Face Look Frozen?
That should not be the objective.
Appropriate treatment can reduce selected muscle activity while retaining expression. The desired amount of movement should form part of the treatment discussion before injections are performed.
Can Botox Make a Man Look Feminine?
Botox itself does not feminise the face.
Poorly planned alterations in brow shape, muscle balance or excessive smoothing can alter facial character, which is why treatment should respect the patient’s own anatomy rather than apply a standard aesthetic template.
Does Botox Cost More for Men?
Not automatically.

However, a slimmer jaw is not automatically desirable for every male face. Muscle size, bone structure and overall facial proportions should be assessed first.
